- How to Schedule an Appointment: Appointment and referral are required. Providers and local County Health Departments must complete a signed Infectious Disease Requisition (IDR) form and contact Warren County Health Services to make an appointment for the client.
- Additional information: Drive-thru site open Monday – Friday, 9-11 am. Please use the Glen Lake Rd. entrance. Those without the appropriate IDR form will be turned away. NO PETS IN CAR & NO SMOKING.

- Additional information: Bring any insurance information. Bring proof of identity and in-state residence, as well as your test confirmation e-mail or text message. Be sure to have your mobile phone in case they need to reach you. Upon arrival stay in your vehicle and follow all posted instructions at the site.

- Criteria for Testing: Provider order required. Emergent Care can order testing when appropriate.
- How to Schedule an Appointment: This is a walk-in establishment; no appointment is necessary. If you do not have a provider, you may book a telehealth online screening to determine if you qualify. Screening is available 24/7.
- Additional information: COVID-19 and antibody testing are available with a provider order 7 AM -7 PM Monday-Saturday, and 7 AM-5 PM on Sunday. You are encouraged to contact your insurance company prior to arrival for coverage information.
