Anyone interested in receiving a vaccine should contact the Health Department at (518) 746-2400 to schedule an appointment. The county will accept walk-ins during clinic hours. But calling for an appointment guarantees they will have a shot waiting for you.

We have included a  fact sheet from the County Health Department on the Pfizer vaccine. You will receive the same information if you visit a clinic that is giving out the Pfizer vaccine.

Additionally, there is a printable form below for people to fill out before they arrive at the vaccine clinic. Parents of 16-to 18-year-old children should fill out the portions highlighted in yellow if they are unable to attend the clinic with their child. The County Health Department would also like a contact number for the parent if they are not going to be present at the clinic with their child.
